Purpose
This assessment allows You to demonstrate you skill in developing a health promotion plan and visually communicating that plan effectively.

Task details
Drawing together information from an Assets and Needs Assessment along with evidence and theory to develop a comprehensive multi-level health
promotion plan is a core competency required for contemporary health promotion practice. The ability to justify a health promotion plan and demonstrate
the range of partners that can contribute to the
implementation of the plan is a skill that is drawn upon regularly amongst health promotion practitioners. Assessment 3 requires students to develop a poster that details a multi-level health
promotion plan to address the health/social issue in a priority population at a municipal, regional, or state level identified in Assessment 1,and drawing on the theory and evidence reviewed in Assessment 1. Students are required to justify the plan proposed
and provide a partnerships map.

Assessment 3 draws on cO ntent provided through Weeks 1-6 of the subject.
You are working as a Health Promotion Officer. You have been asked by your Manager to prepare a poster presenting a multi-level health promotion plan, justificationandpartnershipmap
to your organisations Community
Advisory Committee on the social/health issue, priority population and geographical area that you focused on in Assessment 1and 2. To ensure your information is accessible to all
members
of the Community
Advisory Committee
(including community
members
from diverse backgrounds), you are required to prepare a clear,
succinct, plain language poster.

In preparing your poster you must include the following:
e Theissue
o Clearlyidentifythehealth/socialissue,the
priority population and the geographical area.
A clear statement of the most pressing
needs to be addressed that are amenable to health promotion action (identified in
Assessment 1).
* The program plan
o One goal, three (or more) objectives and
three (or more) strategies are to be presented.
The goal, objectives and strategies should be clearly and logically aligned with the determinants of chosen health/social
issue, the previously identified needs, a theory of change, the evidence and draw on the identified assets. To achieve this,
students should choose and apply one health promotion planning model (see

The goals and objectives should meet the SMART criteria.
© Strategies chosen should adopt a portfolio
approach; that is,each strategy should address a different level of change from a variety of health promotion action areas.
For example, the Ottawa Charter includes five action areas; a portfolio approach to
strategy selection entails adopting a strategy from each of the action areas, rather than selectively focusing on one action area alone.

You can make your poster using any software you like. Suggestions include Powerpoint, Canva, or Microsoft Word. There is a large range of free poster templates available on the internet (see for example, Powerpoint poster templates, or try entering ‘free creative research poster template’ into Google), or you can
create your own.
All information that makes up the poster should be presented on a single slide/page.
A reference listmust be included on the single slide/page submitted, correctly formatted in Vancouver style. In-text citations and the reference listarenotincludedinthewordcount.
The poster should be submitted in electronic form. Acceptable submission formats include PDF, Powerpoint or MS Word.
